Admission Requirements:
- Must meet eligibility requirements per referral
- Must not be dangerous to themselves or others
- Must be 18 years of age
- Must voluntarily agree and be able to participate in all program components.
- Must be able to maintain personal care needs independently or with minimal staff assistance
- Assessments, skills programming, and goal setting
- Must have desire and need for skill building in any of the following areas. Self care, communication, independent supported living, community support.
- Symptoms of illness, physical or mental are as such that they are not
- disruptive to program, or the rehabilitative process.
- Behavorial problems are controllable with minimal staff intervention, and will not cause a threatening or intimidating environment which will interfere with operation of program.
- Persons with a history of intensive criminal background may not be eligible for program.
Permanent Eligibility:
- One six month stay in an inpatient psychiatric unit.
- Two stays of any length in an inpatient psychiatric unit in the preceding 2 years.
- Three or more admissions to an office of mental health operated or licensed certified mental health outpatient program or a forensic satellite unit operated by the office of mental health within the preceding 18 months; or a combination of three admissions or contacts within the preceding 18 months.
- SSI/SSDI recipient due to mental illness.
- Twelve months active enrollment as a waived client
- Six months consecutive residency in a designated adult home.
- Six months consecutive residency in a residential care center for adults.
- Six months consecutive residency in a residential treatment facility.
Categorical Eligibility:
- Resident in a designated adult less than six months.
- Resident in a designated shelter for the homeless.
- Resident in a designated single room occupancy hotel.
- Resident in a community residenece less than six months.
- Resident in a family care home less than six months.
- Resident in a residential care center for adults less than six months.
