YES YOU CAN... The Traumatic Brain Injury Medicaid Waiver Program provides support and assistance so you can live a full, productive and meaningful life. Learn how to focus on your strengths and abilities; with adjustments you can achieve your goals with satisfaction and feelings of accomplishment. You can learn how to remain focused on tasks to completion. You may need assistance to learn how to do things differently, but you can become independent. As a participant in this program, you can discover where you fit-in and feel good about yourself. Accepting yourself as valuable and contributing, you can establish new and meaningful relationships; you can break the bondage of loneliness. The future does hold a place for you, discover it with the assistance this program has to offer you and believe... YES I CAN!

Traumatic Brain Injury Medicaid Waiver Program Services

  • Service Coordination
    Integrating a network of person centered supports and services.
  • Independent Living Skills Training and Development
    Teaching new strategies to achieve you goals, one-to-one.
  • Transportation
    To non-medical appointments for the purpose of community inclusion
  • Structured Day Programs
    Learning new skills with peers
  • Substance Abuse Programs
    Interventions to reduce or eliminate obstructions to community inclusion
  • Intensive Behavioral Programs
    Decrease maladaptive behaviors and substitute socially appropriate interactions
  • Community Integration Counseling
    Effectively manage and overcome the difficulties and stressors of community integration
  • Home and Community Support Services
    Assistance, training and supervision with activities of daily living.
  • Environmental Modifications
    Internal and external home adaptation to ensure health, welfare and safety.
  • Vehicle Modification
    Modifications and adaptations to primary vehicle increasing community accessibility.
  • Special Medical Equipment and Supplies
    Durable medical equipment and supplies not funded through the State Medicaid plan.
